Outstanding Care Homes

Portland care homes are specialists in residential care, nursing care, end of life care, therapy and rehabilitation, dementia care and day, respite and short breaks, providing care environments to support the needs and risks posed.

Portland Care works successfully with dementia, complex needs, residential residents with or without nursing care.

Working with people, their care teams and their loved ones to enable them to leave hospital and live their  life safely with person centred, least restrictive care.

Recreation programs keep residents active and engaged in art, music and other activities, while regular outings, holiday celebrations and other events foster social interaction and a sense of community.

Care plans are tailored to residents’ individual medical and social needs and managed by interdisciplinary teams of professionals, who meet regularly with residents and those who care about them to review their quality of life goals, assess progress and adjust the plan as needed.
